The Forms section is designed to guide you through the home study process with AAA Partners in Adoption (PIA).
The forms are listed in the order that you will need them with the Application being the first. The final form, Checklist of Documents for Homestudy, is
the only form in the list that you will not be required to download, print, complete and submit to PIA at some point in the process. That form is for your
use only in the collection of the supporting documents that you will need to submit during the process.
The printable forms are available to you in
two formats:MSWord form (.doc) and Adobe Acrobat (.pdf). Download these forms into your computer in whichever format you choose, then print them, and fill
them out.. You will need to mail these forms to AAA P.I.A. Along with your application fee. (we do not offer online submission due to laws requiring all
forms to have Original Signatures).
If you do not have MSword then you must use the (.pdf) forms. If you do not have Acrobat Reader,
you can download it for free by clicking HERE.

PLEASE NOTE: It is very important that you
retain copies of all documents provided to AAA Partners
in Adoption, as you will be required to provide them to
your placement agency.
